1. The Student Group Insurance for students of MacKay Medical University is underwritten by Nan Shan Life Insurance Co., Ltd.

2. Required Documents for Insurance Claims:

• Student Group Insurance Claim Application Form (available at the Health Center or downloadable from the Health Center website)

• Original or certified copy of the medical diagnosis certificate (copies must bear the official hospital stamp)

• Original or certified copy of medical receipts (copies must bear the official hospital stamp)

• Photocopy of the applicant's bank passbook cover page showing the account holder's name and account number

• For fracture claims, please provide a CD containing the relevant X-ray images.

3. After preparing all required documents, please submit them to the Health Center for verification during office hours.
